Thank you for your patience during these unprecedented times due to COVID-19.
Safety recommendations continue to be updated and we are following them closely. Being a health facility we will continue to require masks to be worn properly covering your nose and mouth while in our office. We are still limiting the number of people in the office so we ask that you enter alone or if necessary with no more than one person (a caregiver or child)
If you answer YES to any of the following questions we will have to postpone your eye exam until you are feeling better.
In the last 2 weeks:
- Have you had a fever (100.4°F or higher), cough, shortness of breath or chills?
- Have you or someone you are in contact with been diagnosed with COVID-19?
- Have you traveled out of the country?
Upon arrival to the office:
Please call us at 714-521-3002 to notify us you have arrived. One of the staff will come out to your car to take your temperature and gather your paperwork.
If at all possible we ask if you are a New Patient to our office that you download and fill out the Medical History Form under Patient Forms here on our website.
